Sarah L. Masucci, LCSW, LCADC, a Master's Level state licensed social worker with certifications in Gerontology and Clinical Supervision is the Executive Director of Supportive Care Services, LLC. She has over 20 years of experience helping clients with chronic, debilitating medical conditions navigate complex health and home care systems. Sarah specializes in consulting with clients as to how best to use a combination of public, private and family resources with an emphasis on client preference. Her expertise is in defining unmet needs and developing a host of options to support clients and their families in receiving appropriate care at home. Sarah focuses on establishing creative and collaborate relationships with her clients and the community service providers that are incorporated into each client’s plan of care. Her passion is in working with older adults to achieve a level of support that provides the greatest degree of autonomy, functionality, choice and safety. She has experience working in government as well as developing a successful social support program for a large multi-specialty physician group. She understands the importance of building community relationships that help patients get the care they need and deserve.

Ann St. John, MSW, LSW

Ann St. John is a NJ licensed social worker who has spent her 25-year career assisting vulnerable, marginalized and economically disadvantaged adults in Hunterdon County. Ann grew up in Hunterdon County and understands first hand the changing culture of the area and the unique struggles many older adults face in a semi-rural environment. She worked at Hunterdon County Social Services for 18 years in various positions that gave her valuable experience in crisis intervention, Medicaid determinations, case management and placement of emergency services. She came to Supportive Care Services after working as a Care Manager at Hunterdon County Senior, Disabilities and Veteran’s Services. Ann earned her Master's degree in Social Work at Marywood University with a concentration in end-of-life care. Her passion is in being available to older adults and their families when they find themselves experiencing a gap in traditional care services during evening and weekend hours. Ann's dedication to her field has secured her lasting professional relationships with area providers of senior services, medical and legal professionals. Ann is the type of professional who will “go the extra mile” for her clients. Supportive Care Services is fortunate to have her onboard.
